Membership On Hold Request

Please complete this form to request to put your membership on hold.

Membership on hold guidelines:

• The on hold fee is half of your monthly membership cost
• On Hold requests will only be approved for up to 4 consecutive months in a 12-month period.
• If placing your membership on hold for medical reasons, documentation from a physician verifying the need to place the membership on hold must be provided. The membership will then be on hold at no cost for the duration of the medical hold request.
