Private Pickleball Lesson Form

The primary contact will fill out this form, online (preferred) or in person. This form will then be distributed to the Recreation Manager – Athletics. They will then pass along the information to the private lesson instructors to contact the primary contact directly. Please allow at least 2 business days to be contacted.
